Southern New England Practice Transformation Networks

In the fall of 2015 the Center for Medicare & Medicaid Innovation (CMMI) awarded funding to 29 Practice Transformation Networks (PTNs) nationwide. The Southern New England Practice Transformation Network is a collaboration between UMass and UConn Medical Schools, operating primarily in Connecticut and Massachusetts. The Southern New England PTN is recruiting clinicians to participate in this initiative, who are not already participating in a Medicare ACO. 

The PTNs are designed to offer technical assistance, quality improvement services, and to prepare clinical practices to participate in alternative payment methods, at no cost to clinicians.  Benefits of participation include:

  • Practice readiness for new funding models;
  • Increasing practice revenue by utilizing new codes;
  • Optimizing use of health IT and data;
  • Strengthening connections to other providers and community resources;
  • Free quality improvement consultation;
  • Access to resources, community peers and experts; and
  • Improving patient & clinician experience of care.
