APTA CT has 5 Chapter Awards available for APTA CT members. Awards will be announced at our Annual Meeting.
All 5 awards require:
1. Submission Form - see below (Google Form)
2. Letters of recommendation spotlighting the nominees contributions.
3. CV or resume if able - it is always a good idea to reach out to employers as they may have this. Also, if available, a picture of nominee.
Submissions are due: September 12, 2024
